Our private duty nurses offer one-on-one medical care that adapts to your loved one’s changing needs throughout the recovery process. From managing medications and monitoring vital signs to assisting with rehabilitation goals, our nurses provide both clinical expertise and genuine support.
Every nurse in our network is thoroughly trained and certified in specialty areas of care. Our Care Coordinators carefully match each client with a nurse whose skills and experience best align with their needs and preferences—ensuring safe, reliable, and personalized care.

Many stroke survivors experience difficulty swallowing and may require nutritional support. Our nurses are trained to safely administer tube feedings and maintain feeding equipment.
We provide skilled tracheostomy care, including suctioning, cleaning, and monitoring, as well as assistance with other respiratory needs.
Our nurses ensure medications are organized, administered correctly, and adjusted as needed to support recovery.
For stroke patients who have undergone surgery, our nurses provide wound monitoring, dressing changes, and overall site care to promote proper healing
The move from hospital to home can feel overwhelming. Gentle Horizon nurses provide professional support to make the transition smoother and safer.

Stroke survivors can often return home after a hospital stay. At Gentle Horizon, we provide licensed private duty nurses who deliver individualized care in the comfort of your home—or in another safe environment where your loved one feels most supported.
A private nurse can provide skilled, one-on-one care tailored to each patient’s unique needs. Services may include tube feedings, tracheostomy care, medication management, wound care, and other medical support. This personalized attention promotes safety, comfort, and progress during recovery.
Costs vary based on the level of assistance needed, total hours of care, and the nurse’s licensure, training, and experience. We will work with you to develop a care plan that meets your needs while offering transparent pricing.
Currently, Gentle Horizon is private pay. However, we do assist clients in seeking reimbursement from primary health insurance and long-term care insurance. We encourage you to contact your insurance provider directly to confirm eligibility.
Yes. A physician’s order is required for in-home stroke recovery care. While a general nurse assessment does not require authorization, any nursing interventions or treatments do.
