Hospice care is designed for individuals with a life-threatening or terminal illness who are in the final stages of life. To determine if hospice is the right option, it’s important to discuss your situation and available choices with your physician.
Most hospice patients are cared for in the comfort of their own home or the home of a loved one. At Gentle Horizon, we provide private duty nurses who deliver individualized, compassionate support wherever the patient feels most comfortable.
Costs vary based on the level of care needed, the number of service hours, and the nurse’s licensure and experience. Hospice care may last a few days to several months, depending on the patient’s condition. We provide transparent pricing tailored to each family’s needs
At this time, Gentle Horizon is private pay. However, we work with clients to assist in reimbursement from primary health insurance and long-term care insurance. We recommend contacting your insurance provider to confirm eligibility.
Yes. A physician’s order is required to begin in-home hospice care. While a general nursing assessment does not require prior authorization, any nursing treatments or interventions do.
Both services focus on improving quality of life. Hospice care is for patients with advanced, life-limiting illnesses, with the goal of maximizing comfort and dignity at the end of life. Palliative care can be provided at any stage of a serious illness, with the aim of managing symptoms, relieving stress, and supporting both patients and families.
