Post-hospital care allows clients to recover in the comfort and privacy of their own home while receiving one-on-one attention. A private duty nurse or caregiver ensures proper monitoring, support, and personalized care during the transition from hospital to home.
The cost of care depends on the level of support needed, the number of hours required, and the licensure and experience of the nurse or caregiver. Our team will work with you to create a care plan that meets both your medical needs and your budget.
Care can begin as soon as you need it. In many cases, a nurse or caregiver can be present when you arrive home—or even meet you at the hospital to assist during the discharge process and journey home
Yes. A doctor’s order is required for any medical treatment or intervention. However, a nurse assessment, education, or non-medical support does not require authorization. Our team will coordinate with your physician to ensure a safe and compliant care plan
