Palliative Care for Chronic Pain

Palliative care is designed to ease the symptoms, stress, and challenges of living with a serious illness or injury. The goal is to improve comfort and quality of life for both the patient and their family.

Gentle Horizon provides palliative care in the comfort of your home or in another environment where the patient feels most at ease. A private nurse or caregiver can come directly to you, offering individualized and compassionate support.

Palliative care focuses on relieving pain, managing symptoms, and supporting quality of life at any stage of illness—whether short-term, long-term, or alongside treatment. Hospice care, on the other hand, is specifically for individuals with advanced, life-limiting conditions and emphasizes comfort during the final stages of life.

The cost depends on the level of care needed, the number of hours, and whether services are provided by an RN, LPN, CNA, or caregiver. Gentle Horizon will provide a personalized estimate during your consultation so you know exactly what to expect.

Gentle Horizon is currently private pay. However, many long-term care insurance plans may reimburse for palliative care services. We will gladly provide documentation to help you with reimbursement. Please check with your insurance provider to confirm eligibility.

Yes. A doctor’s order is required for nursing treatments such as wound care, IV therapy, or medication administration. However, nurse assessments and non-medical caregiver support do not require a doctor’s authorization.
